There is an opportunity to work in the Hospital Discharge Bureau a busy, friendly, multidisciplinary team managing safe and timely discharges by helping patients assess their social needs and identify support for discharge. We work with people over the age of 18 who have a wide range of medical and social needs, the work is varied and never boring. Case work is usually short term and can be complex, organizational skills and creative thinking will help the post holder progress their career at a fast pace.

Experience of adult social work and assessments would be a bonus however, if the successful candidate is newly qualified, they will join the NQSW programme set up by Southampton City Council where they will gain networking skills and share experiences with other NQSWs.
